New Patients
New patients are those who have never had a medical cannabis card recommendation. All new patients will have to fill out an intake form explaining your medical conditions in which you are seeking a medical cannabis card recommendation for prior to seeing the physician.
Patients seeking a renewal
Renewal patients are those who have had a valid medical cannabis card recommendation in the past year. You must renew your medical cannabis recommendation withing 30 days of expiration in order to be considered a renewal patient. Please bring your expired recommendation with you to your evaluation.
Requirements
All patients must have ONE of the following:

A valid California Identification card or California drivers license

Passport with proof of current California residency

Out of state Identification card with proof of current California residency

Military identification with proof of current California residency
